This form is to be completed each trimester, in addition to NDI New Mexico's Medical and Media release, which are completed upon registration at the start of the school year.

The NDI New Mexico attendance policy states that all students must be signed in and out by a parent/guardian.

I grant my child permission to sign in and out and transport themselves to and from NDI New Mexico and/or to offsite locations.
